It is a form. The fields on the form were filled in with a typewriter and signed by hand. The paper is slightly yellowed.

The passport photo is black and white. Martha Kaufmann is wearing a polka dot shirt with a striped waistcoat. Her hair is loosely pinned back and she is looking directly at the camera with her mouth closed and a slight smile on her face. Her head is tilted slightly to the right.

MARTHA KAUFMANN (née NIMSCHEWSKI) (1899–1941)

Martha Kaufmann came from Gersdorf in Saxony. She lived with her husband, the carpenter Adolf Kaufmann, in Winsen/Luhe. The couple had one child. Martha Kaufmann fell ill at the age of 32 and was admitted to the Lüneburg mental hospital shortly before Christmas 1930. She took her embroidery with her. Six years later, Max Bräuner applied for her to be sterilised. It is almost certain that she was forcibly sterilised in the same year. Four years later, she was registered for »Aktion T4« and transferred to the Herborn intermediate institution on 9 April 1941 as part of the »planned economy«. On 12 May 1941, she was murdered with carbon monoxide at the Hadamar killing centre.
